SUMMARY
The HARA Housing Resource Specialist serves as the point of contact for eligibility determination, case plan development, case management, outcome reporting, and follow up for HARA services offered through TrueNorth Community Services. The Specialist works to identify needs and solutions for low-income households, track outcome data on each household served and use continuous quality improvement methodologies to manage services to outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- In coordination with Homeless Solutions Director and the Housing Services Director will implement, define, and expand the delivery of HARA services to housing services consumers;
- Use skills and experience to conduct and interpret agency defined assessment processes to determine the appropriate service track within housing programs;
- Develop and implement a plan to address household needs and focus on engaging the consumer in a change process toward self-sufficiency;
- Routinely evaluate the progress toward goals identified in each plan of service and modify the plan as needed to encourage ongoing progress toward self-sufficiency and defined program outcomes;
- Use independent judgment and discretion to refer clients to suitable resources and agency partners for needs unmet by TrueNorth;
- Use problem-solving skills to navigate household barriers to safe and stable housing;
- Develop a professional and supportive relationship with each client that establishes trust and serves to motivate toward housing stability and self-sufficiency;
- Advocate on behalf of clients and client rights at local, county-wide, and state platforms;
- Coordinate and participate in local (IST) Inter-agency Service Team meetings
- Enter data into HMIS (Service Point) and/or other HCS relational databases;
- Use established reports from HMIS (Service Point) and/or other relational databases to drive continuous quality improvement processes to improve program outcomes;
- Work closely with HFS agency & system admin to ensure data quality
- Be thoroughly familiar with approved assistance grants, their conditions, and eligibility criteria;
- Create reports on use of programs and grants as assigned;
- Represent TrueNorth at meetings and other gatherings as assigned;
- Travel independently to areas served by TrueNorth, with appropriate compensation;
- Attend mandatory staff meetings; and
- May be asked to assist in any of the five counties TrueNorth serves for housing
- Other duties as assigned.
SUPERVISORY RESPONSIBILITIES: This job has no supervisory responsibilities.
QUALIFICATION/SKILLS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
- Absolute ability to maintain consumer and agency confidentiality;
- Considerable ability to build rapport and maintain professionalism with clients;
- Considerable ability to provide assessment, case management, and follow up services to clients;
- Considerable ability to relate to people on a one on one basis and as a member of a group;
- Considerable ability to effectively communicate with distressed persons under adverse conditions;
- Considerable ability to work independently and as part of team focused on achieving well defined performance outcomes;
- A working knowledge of the barriers and challenges for homeless and/or low-income households;
- Ability to exercise independent judgment and initiative in analyzing consumer problems and recommending appropriate solutions;
- Familiarity and experience with basic computing, such as Microsoft Excel, Outlook, general internet use, and capacity to learn required external databases;
- Ability to problem solve, exercise good discretion and judgment when making independent decisions;
- Possess and demonstrate skill and experience in effectively coordinating multiple projects with short deadlines;
- Ability to work with a variety of systems, agencies, data and individuals while exhibiting excellent communication skills;
- Ability to maintain records and prepare verbal and written reports;
- Ability to explain program policies and procedures to consumers and families of consumers; and
- Ability to establish and maintain effective working relationships with administrators, employees, consumers, and families of consumers.
EDUCATION and/or EXPERIENCE
A Bachelor’s degree is preferred, in the field of health, human services or related field from an accredited university or college AND
- Knowledge of assistance to vulnerable populations, revenue streams and program operation;
- An ability to understand and commit to the agency business model is essential
- At least 2 years of experience working directly with individuals experiencing homelessness
- Ability to work in non-traditional settings and unstructured environments
CERTIFICATES, LICENSES, REGISTRATIONS:
- The employee must possess a valid and unrestricted driver’s license and reliable transportation.
